The French global became the first player in the history of the Premier League to win back-to-back titles with different clubs.

Italian Conte is only the fourth manager to win the Premier League title at the first attempt, emulating his Stamford Bridge predecessors Jose Mourinho and Carlo Ancelotti as well as Manuel Pellegrini who achieved the feat with Manchester City.

Michy Batshuayi scored his second Premier League goal and first since August on his 18th substitute appearance to be the unlikely hero eight minutes from time.

The Mirror is reporting that Lukaku was one of the individuals watching from home, even commenting on the video.

But Belgian Batshuayi had other ideas, scoring in the 82nd minute off a cross from Cesar Azpilicueta. "I am very happy because my dream came true".

Prior to leaving Juve, Conte wanted to change to a 4-2-3-1 formation with Alexis Sanchez and Juan Cuadrado his major targets.

“This is a great achievement for the players, ” Conte told Sky Sports in a pitch-side interview after breaking away from his players jubilant celebrations.

"And in that moment it was frustrating for me and I found the strength to change, to take responsibility, to change system and find a new suit for these players".

Conte, who still remains on course for a famous league and FA Cup double, believes that there is more to come from his side next season and he wants to be a part of the exciting project. "It's important for me and also my players because don't forget that last season was a bad season for the club, the players and the fans".

The Italian, victor of three Serie A titles as Juventus manager, has transformed Chelsea from flops to champions in one season with largely the same group of players. "To find the right solution quickly wasn't easy".

But with an FA Cup final against Arsenal awaiting on May 27, David Luiz says it is vital Chelsea do not allow themselves to take their foot off the pedal.

"I thank them for their commitment and work rate", Conte said of his players.

"I think it's more rumour than reality".

"For this reason certain players didn't have a lot of time but they worked very hard, Michy is a great example".

Conor Gallagher and Harvey St Clair put Chelsea into a commanding lead before captain Mason Mount rounded off the game and the Premier League season with a deft finish.

Adding to his already impressive list of achievements as a manager, Conte says his fierce dedication to the team's success made the match an nearly surreal occasion.
